MISSION
To build an interdisciplinary referral network of providers who specialize in treating patients with chronic pain and dysautonomia conditions.
Vision
To radically transform the treatment of chronic pain and dysautonomia conditions through compassionate, collaborative, interdisciplinary care that improves providers’ knowledge and resources and enhances patient access, support, and outcomes.
Approach
Quarterly gatherings to build connections between Vermont providers across disciplines and practice locations.
Bi-monthly collaborative meetings featuring case studies and roundtables to develop best practices and interdisciplinary approaches to treating chronic pain and dysautonomia conditions.
An online group or listserv to share resources, provide support, and facilitate appropriate referrals to enhance interdisciplinary, multimodal patient care.
Opportunities to collaborate on building patient and provider resources, educational opportunities, and support networks.
